eligibility criteria
To be eligible for financial assistance from Flutter Club, Inc., applicants must meet all of the following criteria:
Residency Requirement
Must reside in one of the following counties:
New York: Albany, Columbia, Delaware, Dutchess, Essex, Fulton, Greene, Hamilton, Herkimer, Montgomery, Otsego, Oneida, Rensselaer, Saratoga, Schenectady, Schoharie, Ulster, Warren, Washington
Massachusetts: Berkshire, Franklin, Hampshire
Vermont: Bennington, WindhamMedical Eligibility
Applicants must have a confirmed diagnosis or medical need from an accredited medical center and be referred by that institution’s licensed clinical social worker or medical staff member.Eligible medical conditions include a:
Confirmed critical illness
Confirmed developmental disability
Confirmed reproductive or fertility-related condition or need
A medically indicated need for a second opinion related to any of the conditions listed above.*
Required Documentation
A signed referral from the patient’s treatment center by a licensed clinical advocate (e.g., social worker, nurse navigator, or physician).
A completed application signed by the patient or legal guardian and properly attested.
*If the applicant requires travel assistance for a second opinion before official confirmed diagnosis, Flutter Club, inc. will financially assist for up to two rides on The Flutter Express. Please indicate this need in the application.

what happens next?
Initial application decisions will be communicated by May 31, 2025. After this date, applicants can expect to receive a decision or next steps within 5–10 business days of their provider’s submission.

Submitting an application does not guarantee approval. All assistance is subject to availability of funds and program capacity. Flutter Club, Inc. reserves the right to request additional documentation, deny applications that do not meet eligibility criteria, or modify program terms at any time.
understanding The Flutter Express
Flutter Club, Inc. provides financial assistance specifically for travel on The Flutter Express. This is a non-emergency, non-medical transportation service—comparable to a traditional motor coach bus used for commuting or entertainment purposes.
Please note:
The Flutter Express is not an ambulance, ambulette, or medical transport vehicle.
It does not include onboard medical personnel, treatment services, or medical equipment.
Riders must be medically stable and able to travel independently or with a caregiver or companion.

At Flutter Club, we support individuals who need to travel for a second medical opinion, as well as those navigating critical illnesses, developmental disabilities, and reproductive health and fertility needs, as defined below.
critical illness
A critical illness is a severe health condition that poses a significant risk to a person's life or long-term well-being. These illnesses often require specialized medical treatment, ongoing care, or travel to world-renowned healthcare facilities.
Some Examples of Critical Illnesses:
Cancer (e.g., leukemia, breast cancer, brain tumors)
Organ Failure (e.g., kidney failure, heart failure, liver failure)
Neurological Disorders (e.g., ALS, multiple sclerosis, severe epilepsy)
Cardiovascular Diseases (e.g., stroke, heart attack, congenital heart conditions)
Severe Respiratory Conditions (e.g., cystic fibrosis, pulmonary hypertension)
Autoimmune Diseases (e.g., lupus, severe rheumatoid arthritis)
Blood Disorders (e.g., aplastic anemia, myelodysplastic syndromes, paroxysmal nocturnal hemoglobinuria)

We also support individuals navigating the complex world of fertility preservation and reproductive healthcare—especially those impacted by health-related barriers like prior cancer treatment, genetic conditions, or reproductive system disorders.
Some examples of Reproductive Health & Fertility Needs:
In Vitro Fertilization (IVF) and other assisted reproductive technologies (ART)
Fertility preservation procedures, such as egg, embryo, or sperm freezing
Reproductive endocrinology consultations and treatment plans
Third-party reproduction support (e.g., egg donor, sperm donor, or gestational carrier coordination)
Preimplantation genetic testing and counseling
Endometriosis and PCOS treatment at specialized centers
Surgical procedures related to reproductive organs (e.g., fibroid removal, laparoscopic surgery, ovarian cyst treatment)
